Great question. I'd decide whether I wanted to do northern or southern Italy. In the north, I'd take in some of the following: Milan, Lake Como, Verona, Venice, Bologna, Florence. In the south, you could also start in Florence, take day trips to places like Siena or Lucca (you could do this in your northern trip too), Rome, Naples, Ischia, Sorrento, the Amalfi Coast, and Capri. These are all just places I've visited, I've almost certainly missed places others would say are must see.
Another funny and witty story that again pulls me in to the travel shop, picking the brochures for Italy. Reading about Sorrento whilst eating a ham sandwich but yearning for fresh pizza.
